If you are a new patient, or looking to become one, the following information is important for you to know:
- New patients should arrive 15 minutes early to allow enough time to check-in. Please bring your current insurance card.
- If you are experiencing a performance related injury, please bring in the shoes you use for training and/or racing.
- If you have received any imaging due to your current injury, please bring any reports and/or films.
- If you are experiencing a lower body injury, please bring a pair of shorts. This allows the injured area to be easily assessable.
- If you are experiencing an upper body injury, please use the same consideration.
- Please do not leave children unattended in the physical therapy area.
- Children are able to been seen by our entire staff with proper parental consent.
- We have weekend hours every Saturday from 8am-12am, with the exception of physical therapy.
- Co-payments are expected at the time of each visit.
- We accept all major credit cards, checks, and money.
Please be advised that appointment times can run late due to the personal attention given to each patient.
For patients that do not have health insurance, we offer a self-pay rate. Initial
consultations are for $65.00 for seniors, and $95.00 for students and adults.
Follow up visits are $35.00 for seniors, and $55.00 for students and adults.
